Summary of the Community Teaching Plan 

The community teaching plan focused on prevention and health promotion in the community. The teaching plan targeted a church community to address the prevention and management of diabetes in the individual and community. The teaching location was at the Redeemed Christian Church of God, Winners Assembly, Bellevue, Nebraska. The aim was to empower individuals and the community to take control of their health by educating them regarding effective diabetes management and prevention strategies. Promoting self-efficacy is critical to promoting a healthy community because national health can only begin at the individual level. The teaching plan also intended to help achieve the Healthy People 2030 goal of reducing the burden of diabetes and enhancing the quality of life for everyone with or who is at risk for diabetes (Centers for Disease Control and Prevention, 2022). Diabetes is a prevalent health condition in the United States, affecting about 37.3 million people of all ages, 11.3% of the population (National Institute of Diabetes and Digestive and Kidney Diseases [NIDDK], 2023). Around 37.1 million people with diabetes are those 18 years and above. Diabetes is often underdiagnosed because, according to the NIDDK (2023), only 28.7 million people of all ages have been diagnosed with it, about 8.7%. Children diagnosed with diabetes are 283,000, and young people below age 20 with type 1 diabetes are 244,000 (NIDDK, 2023). About 8.5 million people 18 years or older have diabetes but are yet to be diagnosed, which is about 23% of all adults with diabetes (NIDDK, 2023). Evaluation of Teaching Experience 

The physical assessment and the interview were successful, and they generated vital data reflecting the community’s health status, health issues, and needs and what is being done now to prevent and manage diabetes in the community, especially among low-income individuals. The teaching was insightful and inspiring for the local community, who realized they had more power over their health than they thought, eliminating the rationale that the government is in charge of their health and that every health issue prevalent in the community is because the government or the healthcare system failed. Community Response to Teaching 

The church community received the community teaching initiative respectfully. It acknowledged the increased need for more public health education programs to promote health literacy and enhance awareness of community health issues. The community was more knowledgeable of the connection between lifestyles and health status, and they needed education on gaining control of their health and taking the initiative to ensure a healthy community. Everyone, including the audience, played their role in actualizing the community teaching plan, which is encouraging because it implies more people want to be in the driving seat of their health and take more accountability. Areas of Strength and Areas for Improvement 

The most fundamental strength is the teaching achieving its objective. The audience felt the teaching targeted their health needs and issues, increasing interest in participation and engagement.
