Review of Current Healthcare Issues

While the US has made several improvements in healthcare, only 10% of Americans are satisfied with the healthcare system in general (AAMC, 2020). In this regard, seven in ten people interviewed believe that the healthcare system has several major problems. One of the current issues in US healthcare today is the shortage of nurses and physicians.

According to the Association of American Medical Colleges (AAMC), there will be a shortage of approximately 54,100 and 139,000 physicians as of 2033, while the demand for these providers is set to increase (AAMC, 2020). Nonetheless, the organization reported that despite a 16% increase in the graduate medical slot over the previous years, the supply of physicians will be projected to increase by only 0.5% per year between 2016 and 2030.

Nurse Practitioners (NPs) make up the majority of the APRNs and provide care in collaboration with other primary and specialized physicians. NPs are increasing rapidly due to shorter training times and have fewer institutional constraints, unlike physicians. The imbalance between these providers has a long-lasting strain on physicians’ demands. Nonetheless, patients continue to visit healthcare facilities seeking primary care services, making it difficult for a single healthcare provider to address these needs effectively.

While NPs have the skills to perform several patient care duties, most patients and physicians tend to misunderstand their abilities, affecting their overall productivity. For this reason, there is a need to increase training and prepare the NPs for this reality. Auerbach et al. (2018) highlight that NPs provide similar care as physicians provide, which places them in a position to address the patient’s needs.

In this regard, a possible approach to reducing physician demand is to create the right environment in which nurse practitioners can also provide patient care. Evidence shows that primary care provided by NPs is slightly lower than that provided by physicians (Auerbach et al., 2018). This strategy would ensure a balanced ratio of health providers to patients. As a result, there will be increased patient satisfaction, quality care, and appropriate working conditions for the health providers.

Our facility has taken the initiative to enhance co-management between physicians and NPs to reduce strains on physicians. While it provides both primary and specialized care, there is huge traffic from patients seeking outpatient and inpatient care. However, it has few specialized physicians, including pediatricians, gynecologists, and gastroenterologists. They are only available during specific days of the week, where they attend to patients on an appointment basis.

However, they are patients in critical conditions, and at times, they are asked to return to the facility after several days, which poses a risk to their condition. Additionally, these specialized providers are overwhelmed with the number of patients they serve in a day, which reduces their ability to provide care efficiently. Some work in multiple facilities due to shortage issues which leaves them overworked.

After several complaints from these physicians, the facility’s management adopted a team-based model where NPs would work collaboratively with the physicians to provide care to patients. Specifically, NPs were expected to diagnose and provide patient care and appropriate treatment. Through collaboration with other physicians, tasks are shared, reducing the workload of individual providers (Norful et al., 2018).

Adoption of this model has helped to meet the needs of patients while ensuring effective working conditions of the providers. This model has been effective due to established policies in the facility, mutual communication, trust, and collaborative culture (Broome & Marshall, 2021). In this regard, the patients do not have to wait till the specialized provider returns to the clinic to receive care. The NP assigned the work can continue care from where the specialized provider left based on the patient’s medical records, improving overall patient satisfaction.
