CASE STUDY 3

A 27-year-old man with Crohn’s disease has been admitted to the emergency room with an extreme flare-up of his condition. He explains that he has not been able to afford his medications for the last few months and is concerned about the costs he may incur for treatment.

You have a written assignment. Select one of the cases located in the announcements area and write a one-page narrative (not a formal paper) explaining the health assessment information required for a diagnosis of your selected patient (include the case number). Explain how you would respond to the scenario as an advanced practice nurse using evidence-based practice guidelines and applying ethical considerations. Justify your response using at least 3 different references from current evidence based literature.

Crohn’s disease involves bowel inflammation. Information about symptoms, the potential cause of the extreme flare-up, possible causes of the disease, any recent diagnosis, and alternative forms of medication would be vital for the health assessment. Flare-ups are temporary and involve increased symptoms’ intensity. A flare-up can occur if the patient is not under any medication or has received the wrong prescriptions for the condition. Stress can also increase symptom intensity, especially for individuals with Crohn’s disease (Gonzalez‐Montpetit et al., 2020). In this case, the patient cannot afford medication nor the treatment for his condition. This necessitates a care plan that addresses the symptoms while also recommending effective management strategies.(Crohn’s Disease Case Study)

The second step is conducting a diagnosis, and I would ask about the symptoms he is experiencing, and when they started. Symptoms can range from abdominal pain, loss of appetite, diarrhea, fatigue, fever, blood in stool, joints and skin inflammation, perianal fistula, ulcers, and shortness of breath (Malarcher et al., 2017). It is also crucial to determine other factors leading to the severity of the symptoms such as smoking, diet, age, length of time he has had the disease, and rectum involvement (Kaplan, & Ng, 2017). The patient is not under medication, but he might be undergoing other forms of home treatment.

Therefore, I would ask about other possible medications or treatments he might be taking. I would also try to determine the cause of the disease by asking about family history since Crohn’s disease can be hereditary. The immune system, genes, and the environment can be causative factors (Lewis, & Abreu, 2017). Blood tests, stool tests, endoscopy, colonoscopy, and CT scans are also vital for the assessment (Sarmento, 2017). They provide evidence about possible disease causes and the severity of the disease and its symptoms. The health assessment would guide in medical prescription and treatment. Anti-diarrheal, antibiotics, immunomodulators, anti-inflammatory drugs, biological therapies, and dietary changes would be helpful.
